Природа тревожности.
Обзор 13 многомерных анализов, охватывающих
814 переменных (The nature of anxiety: a review of thirteen
(multivariate analyses comprising 814 variables// Psychiatric
Reports, декабрь 1958 2.)

структурами мозга, как миндалевидное тело, гиппокамп, голу-
бое пятно, передняя поясная кора и островок, а также нейро-
медиаторами _ серотонин, норзпинефрин, дофамин, глута-
мат, гамма-аминомасляная кислота (ГАМК) и нейропептид Y
(HHY) — и генетикой, которая все это кодирует. Лечение часто
проводится с помощью медикаментов.
I/I наконец, направление, которое доктор В. называет эмпи-
рическим. Его представители придерживаются экзистенци-
ального принципа, считая панические атаки и навязчивое
беспокойство защитными механизмами, которые психика
вырабатывает при угрозе ее цельности или самооценке. Эмпи-
рический подход, как и психоаналитический, большое значе-
ние придает сути и содержанию тревоги (в отличие от био-
медицинского и поведенческого подходов, занимающихся ее

мам или убежденности в тщете собственного существования.
Лечение предполагает управляемую релаксацию для уменьше-
ния симптомов тревоги, а также помощь пациенту в изучении
его тревог с целью обращения к стоящим за ними экзистенци-
альным проблемам.

психиатрами с медицинским дипломом и психологами-гума-

ложительной эмоции, чувствовали радость, тогда как получавшие повод для не-
гативных эмоций испытывали злость или тревогу, в каждом случае ощущая соот-
ветствующую эмоцию сильнее тех, кто вместо адреналиновой инъекции получил
пустышку-плацебо. Адреналин усиливал интенсивность эмоции, однако не опре-
делял ее характер _ характер зависел от экспериментального контекста. Это зна-
чит, что вегетативная система организма обеспечивает механическую составля-
ющую эмоции, тогда как ее смысловое наполнение обеспечивает интерпретация
окружающей обстановки.
Другие недавние исследования подтверждают правоту предположения
Джеймса и Ланге, что именно физиологические процессы организма управля-
ют эмоциями и определяют их остроту. В частности, все больше научных работ
свидетельствует о том, что мимика способна вызывать, а не только отражать ас-
социируемые с тем или иным выражением лица эмоции. Улыбнитесь _ и почув-

отступить, поскольку осознание, что выход есть, давало бы ощущение контроля

Доктор В., как и Фрейд, полагает, что тревожность может
представлять собой адаптационный механизм, призванный
защитить психику от иных источников печали или боли. Поче-
му в таком случае, спрашиваю я, тревожность часто ощущает-
ся куда острее, чем печаль? Эта душевная «рана», которую мы
якобы нащупали, хоть и доводит меня до слез, но мучает мень-
ше, чем ужас, который охватывает меня в зоне турбулентности
или при тошноте, и чем сепарационная тревожность, накры-
вавшая меня в детстве.
— Так часто бывает, — говорит доктор B.

щих выходных, но тут меня осенило: «B воскресенье вечером все начнется по но-
вой» _ и я оцепенел от бесконечности этой пытки, от ощущения, что воскресные
вечера и утра понедельников будут повторяться снова и снова, и только смерть
положит им конец, а значит, никакого света в конце тоннеля, никакой надежды
на избавление от страха перед предстоящими напастями.
Наглядное свидетельство остроты моей фобии: даже сегодня, 30 лет спу-
стя, я все еще помню тот разговор почти дословно.

В довершение всего мой фобический страх рвоты как раз в это время по-
родил дополнительную боязнь поперхнуться: мне стало трудно глотать. (Трудности
при глотании известны как симптом тревожности по крайней мере с конца XIX B.
и официально называются дисфагией.) Я стал бояться есть. Мое тощее подростко-
вое тело, с каждым днем все больше худея от нервного напряжения, превратилось
в скелет. Я перестал обедать в школе. Чем труднее мне было глотать, тем больше
меня это пугало и тем сильнее обострялась боязнь. Вскоре мне стало трудно глотать
даже слюну. Я сидел на уроке истории с полным ртом слюны и боялся, что сейчас
меня вызовут _ и я либо подавлюсь, либо залью слюной парту (либо и то и другое).
Я стал носить с собой скомканные бумажные платки и потихоньку сливать слюну
в них, чтобы не глотать. К обеденной перемене у меня накапливались полные кар-
маны мокрых платков, которые протекали на штаны, и те воняли слюной. За день
платки расползались в клочья и к вечеру лезли у меня из карманов.
Удивительно, правда, что за все средние и старшие классы у меня было всего
одно свидание?

Примитивные исследования химии мозга английские физи-
ологи проводили еще B конце ХІХ в. Но лишь в 192О-х гг. Отто
Леви, профессору фармакологии австрийского Грацского уни-
верситета, удалось изолировать первый нейромедиатор. В сво-
ей работе 1926 г. он доказывал, что посредником при переда-
че сигналов от одного нервного окончания к другому служит
химическое вещество ацетилхолин*.
Несмотря на рост продаж торазина и милтауна, понятие ней-
ромедиатора — химического вещества, передающего нервные
импульсы между клетками мозга, _ еще не сформировалось”
(Психиатры, назначавшие медикаменты, и даже биохимики,
их разрабатывавшие, в общем и целом слабо представляли,
почему эти лекарства действуют именно так.) Однако откры-
тия двух шотландских ученых обеспечили «суповикам» уверен-
ный перевес в споре. В 1954 г. немецкий нейробиолог Марта
Фогт получила в Эдинбургском университете первое убеди-
тельное доказательство существования нейромедиаторов —
это был норэпинефрин. В том же году коллега Фогт, Джон Ген-
ри Гаддум, B серии нетривиальных экспериментов обнаружил,
что серотонин, который до той поры считался кишечным веще-
ством, участвующим в пищеварении, тоже выступает нейро-
медиатором*""^"*. Гаддум принимал ЛСД, от которого, по его отче-

Связь между вариацией КОМТ met/met и необычно высоким уровнем
тревожности подтверждают многие исследования, правда, почему-то в основном
у женщин. В одном из экспериментов, проведенных учеными Национального
института злоупотребления алкоголем и алкоголизма, участвовали две разные
группы женщин: белые европеоиды из мэрилендского пригорода и индеанки
из сельской Оклахомы (Enoch et al., “Genetic Origins of Anxiety in Women: A Role
for a Functional Catechol-o-methyltransferase Polymorphism,” Psychiatric Genetics 13,
no.1 (2003):33—41). B обеих группах у женщин с вариацией met/met уровень тре-
вожности оказался намного выше, чем у носительниц других вариаций. (Кроме
того, у носительниц met/ met содержание катехол-О-метилтрансферазы в мозге
составляло лишь от четверти до трети нормы.) На ЭЭГ у женщин с met/met OT-
мечался «низковольтный рисунок альфа-волн мозга», который ассоциируется
и с тревожными расстройствами, и с алкоголизмом. Иными словами, исследова-
ние выявило связь не только между геном и уровнем фермента и между уровнем
фермента и активностью мозга, но и между активностью мозга и субъективно
оцениваемым уровнем тревожности. В ходе еще одного исследования, проведен-
ного в 2009 г. с участием немцев и американцев, носители вариации met/met вы-
давали повышенную физиологическую реакцию испуга при показе серии непри-
ятных изображений и, по результатам стандартных личностных тестов, проявляли
повышенную общую тревожность (Armbruster et al., “Variation in Genes Involved in
Dopamine Clearance Influence the Startle Response in Older Adults,” Journal of Neural
Transmission 118, no.9 (2011):1281—92).

Я бы с радостью променял на яму со змеями (неядовитыми) или крысами попада-
ние даже B слабую турбулентность.

Как пишет его лечащий психиатр, «в беседах с ним я делал большой упор
на его личные заслуги перед университетом. С моей помощью он стал находить
большее удовлетворение в своих административных и педагогических достиже-
ниях. В результате самокритичное отношение удалось несколько ослабить».
29 апреля 1949 г. Честера снова вверили заботам жены и домашнего врача,
доктора Ли. В заключении говорилось: «Некоторые признаки напряжения и де-
прессии еще наблюдаются, однако улучшения позволяют выписать пациента».

«Она слишком сильно тревожилась сама и поэтому не мог-
ла быть тебе подмогой в борьбе с тревожностью. Она всю свою
жизнь подчинила избеганию страхов, и поэтому, когда ты впа-
дал в тревогу, она впадала в тревогу вместе с тобой. В такой
детско-родительской связке ребенок перенимает тревож-
ность взрослого, не понимая, откуда эта тревожность берет-
ся. Ее страхи стали твоими, ты не мог с ними справиться, а она
не могла тебе помочь».
«А еще у тебя были сложности с “удержанием объекта”,
-
продолжил он. — Ты не мог удержать мысленный образ роди-
телей. Разлука порождала у тебя фундаментальное сомнение _
вдруг они тебя бросили навсегда. Твои родители никак не мог-
ли остепениться наконец и избавить тебя от этих сомнений››*.
По мнению доктора Л., сепарационную тревожность усугу-
бляла гиперопека со стороны моей матери. «Она подавала тебе
сигналы: “Ты не справишься _ He рискуй, иначе захлебнешь-
ся тревогой”››.
Я замечаю, что доктор Л. объясняет мою тревожность
в основном психодинамическими причинами, взаимоотно-
шениями с родителями. Но ведь современные исследования
показывают, что предрасположенность к тревожности имеет
в основном генетическую природу. Взять хотя бы работы Дже-
рома Кагана, посвященные взаимосвязи генов и темперамен-
вает меня даже в кругу родных и знакомых, и в тот момент я чувствовал себя оди-
ноким как никогда. (В подтверждение привожу запись из отцовского дневника,
который он начал вести после ухода мамы и несколько лет назад любезно дал мне
прочитать: «Однако лет в 11 у Скотта начали появляться разные страхи, сильнее
всего он боялся рвоты. У него возникли некоторые странности поведения, кото-
рые Анна уловила, а я отрицал. Анна была права, доктор Шерри (детский психи-
атр), ругая на чем свет стоит мою психологическую слепоту, рекомендовал обра-
титься в Маклин. В результате Скотт до сих пор проходит психотерапию у доктора
[IL] Однако поначалу был кошмар. Скотту стало гораздо хуже, в частности, он
не мог спать по ночам. Приходилось принимать торазин и имипрамин. Я от от-
чаяния срывался на нем словесно и физически».
