Причина множественной миеломы у животных-компаньонов в
значительной степени неизвестна. У людей заболевания
плазматических клеток связаны с работой в
сельскохозяйственной промышленности, воздействием
нефтепродуктов и хроническим воздействием антигенного
стимула.8-11 На молекулярном уровне множественная миелома
была связана с измененной экспрессией онкогена c-myc (люди) и
белка клеточного цикла циклина D (люди и собаки).12-14 Один
отчет о случае с участием братьев и сестер кошек с
множественной миеломой предполагает, что может существовать
генетическая предрасположенность.5 В отличие от других
гемопоэтических заболеваний, таких как лимфома, нет
доказательств того, что кошачьи вирус иммунодефицита, вирус
лейкемии кошек или вирусные инфекции инфекционного
перитонита кошек связаны с развитием множественной миеломы
у кошек.7
ПАТОЛОГИЯ
Патологические эффекты
Гиперкальциемия. Гиперкальциемия в случаях
множественной миеломы может быть результатом
остеокластической резорбции кости, злокачественной
гиперкальциемии или гиперглобулинемии. Костные запасы
кальция могут высвобождаться остеокластами вторично по
отношению к секреции цитокинов клетками миеломы
(например лимфотоксин, фактор некроза опухоли альфа,
интерлейкины 1, 3 и 6).18 Клетки миеломы также могут
секретировать пептид, связанный с паратиреоидным гормоном,
что приводит к злокачественной паранеопластической
гиперкальциемии.2 Гиперглобулинемия приводит к связыванию
кальция парапротеином, увеличивая общую концентрацию
кальция, в то время как концентрация ионизированного кальция
остается нормальной. Поэтому измерение ионизированного
кальция необходимо для подтверждения истинной
гиперкальциемии у пациентов с множественной миеломой 22.
